Madden Accident Analysis & Forensic Engineering, during a more than 30 year practice in Forensic Engineering, has investigated, analyzed and reconstructed Workplace Accidents in Ohio, Pennsylvania, and other nearby states, and a num-ber of other states from Utah to Pennsylvania. We perform work for plaintiffs and defendants, in civil and criminal litigation, and other dispute resolution. We are available for forensic engineering work throughout the United States. We are currently available for Workplace Accident cases. 
We have analyzed accidents at workplaces for various industrial operations and commercial facilities, as well as accidents at construction sites, drilling and min-ing operations, forestry and agricultural sites, and other outdoor workplace loca-tions. These workplace accidents have included accidents involving machinery, mechanical equipment, combustion equipment, and various components of the workplace facility or location. These workplace accidents have also included accidents involving contact with, and/or escape of, chemicals, gases or other hazardous materials, with or without fire or explosion. Our engineering analysis includes consideration of the relation of the accident to injury or death, whether from physical trauma or contact with chemicals, gases (e.g. carbon monoxide), or other hazardous materials, or from fire and/or explosion. Our engineering an-alysis includes accident analysis and accident reconstruction, and analysis of the safety of equipment, materials, plants, factories, and other facilities and loca-tions involved in workplace accidents. In particular, our engineering analysis includes evaluation of: (a) the use of proper safety design principles; (b) the use of proper machine guards, safeguards, and warnings; and, (c) the adherence to the requirements of code and standards for the equipment, facilities, and materi-als used at the workplace. Our engineering analysis includes consideration of: (a) human factors; (b) the actions of persons in accidents; (c) the policies and procedures controlling and influencing personnel activities; and, (d) the opera-tion, and failures, of machinery and equipment, and failures of structure, and other components of the workplace facility or location. 
In regard to the results of our work, we provide consultation, expert reports, affi-davits and testimony. Testimony has been provided in depositions, arbitrations, hearings, and in trials, including in courts in Ohio, Pennsylvania, and in a num-ber of other states from Kansas to Pennsylvania. 
Investigations and inspections are a part of our engineering analysis of work-place accidents. This includes inspections of the sites of accidents, and also in-spections of machinery, equipment, materials, buildings, plants, factories, pro-perties, etc. involved in accidents, through hands-on inspections, and also through the study of photographic and video documentation of accident sites and machinery, equipment, facilities, etc., involved in accidents. The hands-on inspections augment (and are integrated with) the evidence otherwise docu-mented for the accident. Also included in the investigations are investigations of processes, procedures and activities involved in accidents. Forensic engineer-ing analysis and accident reconstruction have been performed, as appropriate, in the vicinity of the accidents, and also at our offices in the Akron and Cleve-land, Ohio areas.
